CAR T-Cell Therapy Shows Promise for Severe Rheumatoid Arthritis

Rheumatoid arthritis is a chronic autoimmune condition that can cause persistent joint inflammation, pain, swelling, stiffness, and reduced mobility. While modern medications can help control the disease, some people continue to experience severe symptoms despite trying multiple treatments.

CAR T-Cell Therapy Shows Promise for Severe Rheumatoid Arthritis

A recent clinical trial from Charité – Universitätsmedizin Berlin has opened an intriguing new possibility: using CAR T-cell therapy, a treatment originally developed for cancer, to target the immune cells involved in rheumatoid arthritis.

According to a recent report on CAR T-cell therapy for severe rheumatoid arthritis, the early findings suggest that this approach may significantly reduce disease activity and, in some patients, lead to medication-free remission.

What Is Rheumatoid Arthritis?

Rheumatoid arthritis occurs when the immune system mistakenly attacks the body's own joints. Repeated inflammation can cause swelling, pain, stiffness, and progressive joint damage.

For many patients, medications can effectively control inflammation. However, some people develop treatment-refractory rheumatoid arthritis, meaning their symptoms remain inadequately controlled despite trying several therapies.

Could CAR T-Cell Therapy Reset the Immune System?

CAR T-cell therapy is best known as an advanced cancer treatment. It involves collecting a patient's own T cells, genetically modifying them in a laboratory, and giving them a receptor that allows them to recognize a specific target.

In rheumatoid arthritis, researchers are investigating whether this technology can be redirected toward harmful B cells instead of cancer cells. A key target is CD19, a surface molecule found on many B cells.

The goal is to remove disease-driving B cells and potentially allow the immune system to rebuild with fewer harmful immune cells.

What Did the Study Find?

Researchers at Charité tested CD19 CAR T-cell therapy in six people with particularly severe rheumatoid arthritis. All six had previously received multiple targeted or biologic treatments without adequate disease control.

The early results were encouraging. Disease activity decreased substantially in all six participants, and three remained in remission without rheumatoid arthritis medication during follow-up of up to one year.

Why Are These Findings Important?

Researchers found evidence that the modified CAR T cells could reach disease-promoting B cells in deeper locations, including the bone marrow, lymph nodes, and joint tissue.

Rheumatoid arthritis-associated autoantibody levels also dropped sharply during follow-up. When B cells later recovered, predominantly naïve B cells returned, while the disease-associated B cells present before treatment were no longer detectable in almost all patients.

These observations support the researchers' theory that CAR T-cell therapy may potentially reset abnormal immune memory.

Is CAR T-Cell Therapy Safe?

The treatment remains experimental, and its long-term safety is not yet known. In the initial study, all participants experienced temporary mild-to-moderate cytokine release syndrome, which researchers reported was manageable. No severe neurological complications or other serious adverse events were observed, and infections were rare.

What Happens Next?

The next phase of the COMPARE trial is expected to include 10 additional patients. Researchers plan to compare CAR T-cell therapy with an approved rheumatoid arthritis treatment that also targets B cells.

Larger studies will be important for determining whether the treatment can provide durable remission and whether it truly produces a lasting reset of pathological immune memory.
